where's the questionaire?

I finally got my genographic test kit that I ordered from National Geographic. They were on back order for a short while.

There's a bunch of materials, but I thought there was also going to be a 10 or so question optional questionaire about family backround. Perhaps that will be on-line instead?

If anyone has any relevent knowledge or experience, please let me know.
