Failed test

Vial 1 delivered 1st time results for 1st and 3rd Ystr panels and snp test, but it failed 3 times on my second panel Ystr. My second vial now appears to have also failed on that same panel.

Has anyone else had a ystr test fail after 4 testings?
Any thoughts as to why this is happening?
