All three "thirds" failed with the A swab and the first "third" failed with the first try of the B swab. My surname project leader has suggested that he have FTDNA send me a swab kit straight away and not wait for the rest to fail so that they will have a new sample as soon as possible when the other two thirds fail....he also suggested that the 37- to 67-marker test may not be as sensitive as the earlier tests and take alot of sample...sounds plausible to me.
here is what was provided to me...
Y-Refine37to67 146 No results with A Swab - 3 attempts
Y-Refine37to67 162 B Swab - 1st pass failed

Out-of-range values?
I asked FTDNA why my, and my cousin's, 48-60 panel is so delayed. (We have already gotten the 38-47 and 61-67 panels.) I specifically asked whether the reason was a Null value at DYS425. This was the response:

No, both of these samples are in a quality control process because they have an out-of-range value for at least one marker. In order to ensure accuracy with this type of unusual result, we have the lab check over the results an extra time to confirm that these values are reported correctly before they are posted. Tests in this situation are generally resolved within a few days.

The odd thing is that my cousin and I are in different haplogroups! We are not paternally related, except in the sense that our paternal lines both come from rural southern Poland.

In the case of my R1b cousin (YX8BS on Ysearch), the out-of-range value was apparently DYS436=13. Little's list of R1b frequencies does not show this value as ever occurring before in R1b:

In the case of myself, an I1b (HZU4N on Ysearch), the supposedly out-of-range value is unclear. Perhaps it was indeed found incorrect and adjusted.

Seriously, the most common values for those markers in the N Y-DNA Project are just what you have. All the more reason to ask Leah to extend the ranges.
